As I said it could be lots of things. The system is adding fuel to compensate for an imbalance. It doesn't necessarily mean the mixture is lean although it could be. There's not much harm in this other than it may slightly effect fuel economy. First clear the learned value and see what happens...

The oil filter could have gone into bypass but studies have shown full flow filters don't do much of anything anyway. It's what makes the huge marketing behind them so amusing. A good air filter is far more important to the engine's health. That's my story and I'm sticking to it ;)
It does but that's easy for most oil filters, especially if they're changed too often which is what most people do. The tip off would be compressor blade leading edge erosion like the kind typically seen with K & N air filters.
