Make sure the doctor knows it happened at work. There is paperwork to fill out for an occupational injury...do it. Also check to see if there are provisions for you to get paid if you have to take time off. If it's a torn ligament or tendon, you will need time...
You're smart to have told your boss. This would fall under "occupational injury"...there should be provisions at your work to pay for you to see a doctor at no cost to you. I would do so ;)
Measure the head thickness as well...like I said, 116mm is stock.
According to the figures you've posted it looks like a 1.6mm MHG is what you need. Stock HG = 1.37mm (according to IJ), add to that .25mm for decking the head. This assumes this is the 1st time the head was decked.

The "Tom Kitty" was underpowered with the TF30 engines. Very easy to beat down on energy...as soon as the wings spread (they're automatic), a smile would come on my face...
If you're set on using an external wastegate, you could use the top flange in the pic MDC showed (the one w/ 2 pipes) and route the bottom pipe down to your wastegate. A lot like Randy's DDPs...in fact, he could likely custom make you one if you supplied the turbo flange and wastegate/flanges...
