I had that happen for a while and it was the white plastic 'cap' on the bottom of the shifter. Mine was gone and installing a new one along with a new shifter bushing cleared it right up.
Your mileage on this may vary and it may only hold off the need to rebuild the tranny.

Black smoke would be way too much fuel. Leaky injectors, o2, or vac leak.
Smoke from oil cap could be rings or clogged/incorrect pcv.
I'd still do a compression check.

Just be sure to use the stock 'washers'. The rubber is to allow a small amount of flex so the 'tabs' don't break (learned my lesson the hard way... twice).

Valve cover gasket surfaces do not need to be as perfect as head gasket surfaces. You can get away with just using a gasket scraper and making sure they're clean. Slight pitting won't make a difference.

The hp gain is minimal (.5-2hp range) and the stock setup cools better than 90% of the electric fans out there.
If you want better cooling go with an upgraded radiator.
