I've been very happy with my Forge Motorsports FMDV006A. It's a piston type BPV so there's no diaphragm to tear and most places sell it with the spring kit.

If it used to be: Turbo -> Hose -> T -> Hose -> Actuator And it's supposed to be: Turbo -> Hose -> Actuator Then you will have a leftover T and Hose.

Plan on ~2 hours and taking the radiator out (makes the job soooo much easier). Good time to change the belts too.
