There are plastic clips that are slid onto the clip retainers behind the molding, unless those retainers were ripped off the molding, then you would probably need to replace the molding, or push in new ones and then melt the holding tabs onto the retainer so it doesnt fall off.

Most of the time it would be the switches that are corroded/dirty, try fiddling with the passenger side switch and clean the contacts inside the switch, if that doesnt do it, then move on to the other switch on the drivers door.
So it's a non-toyota application? Im not entirely sure about that then, but if you are running a 1j/2jz engine mated with an R154 i don't see why not, it shares the same drivetrain just a different chassis..
Bump, try unlplugging the trans control unit on the passenger kick panel, if you can shift into reverse, and forward gears (1,2, D) then your module is shot, if it doesnt work still then its in the transmission..
