Neglect. Previous owner didn't change the oil/transmission fluid in over a year that he owned it. Who knows how long the owner before him went without changing it either.. ugh.

I've also heard that the small diameter ones clamp shut under vacuum. True? 'Cause I'd like to replace some of the hoses with silicone ones.

Kind of obvious thing happened to mine. Blown head gasket on a 7MGE that had 315xxx KMs on it trying to race my cousins dad driving a 99 dodge ram 3500 dually for fun. Now I am driving a JDM 1G-GTE supra with 120xxx KMs on it and it is still going strong with nothing but a couple small hiccups.

Spun bearing. Previous owner had blown the headgasket several times in the years he owned it and never did anything to the bottom end. When I bought the car there was a quart of oil sitting in the engine compartment so I knew it was coming.
