the best shift knob is whatever you like personally. you can get the tap and die kits cheap, you dont need every possible thread size known to man. in order to find out the thread pitch and everything else of the stock knob you use the gauge that comes with the kit to measure the thread.

forget set screw knobs, they suck. get whatever knob you want thats threaded. figure out what thread the knob accepts, and thread the stick for the knob. if you have too you can get a piece of pipe to use as an adapter if the remaining stick is too small.
since thats all it costs from toyota its worth it to get the toyota replacement. but i must note that the autozone near me (new rochelle..) does carry large ID PCV hoses. so they are available for other things you may need (like power steering reservior lines)

why dont you just bring your old one to an auto parts store and find a hose the right size? it does bend alot but id bet you can use a regular pieces of PCV hose with little problems.
