Tin Snips are your friend. Sears has a 3 piece set for $15. I just used a sharp punch and a piece of wood to 'punch' the holes for bolts to pass thru.
Galvanized sheet metal from Lowe's is like $6.
If you factory system is working properly (no BHG) you won't have (or get) air bubbles in the system.
Once you burp the system correctly the first, your done. I replaced my head gasket 1 year ago. No bubbles, no 'waterfall' sounds, etc.

There is no 'code' for a bad cat. That started with OBD II system in '96. The 02 after the cat measures cat efficiency.
California car? It uses an EGR temp sensor. maybe ??? that?? bad??? sets the code??

Dealer parts. Left side & right side. Cheap. I just got new sets waiting to use for when I go into door to clean and lube all the window parts in there.
PS...use synthetic brake caliper grease for lube, it will last (stay slippery) the rest of the cars life.

Yes, originally it was a threaded hole. I've read on here or other sites about plugs that were stuck so bad they ruined them coming out. That hole sure looks smooth at the thread area.

Mine is original (I believe) with 165k. I recommend changing the cap and rotor every 15k miles. They are cheap insurance for better performance.
