I would definately do a boost leak test to make sure you don't have one. Your turbo should spool way higher than that, and I've always heard good things about your boost controller.

Did you adjust the gain up? If you don't turn the gain up, it won't boost but so high. What are your setting set at right now? I have the AVC-R, but I'm pretty sure it's the same principal. You have a duty cycle and a gain if I'm not mistaken.

There is no other way. If you tighten up the bolts, it won't come off. For the record, I've lost two of them. Both times it happened when I reached eight mph and both times nobody was behind me and both times they were fubar'd.
