The noise you describe sounds like it may be the input shaft bearing. When you hold the clutch pedal down, the trans shaft stops turning, noise gone.
You'll end up putting the 02 bung on the single pipe, not the header. You want it to pick up from 'all' the exhaust. No big deal, we add a bung about once a week at the shop.

That is a coolant hose. It leads down to a fitting on the block under the intake manifold. Its purpose is to pass coolant thru the throttlebody to warm it up. Then the coolant goes to the ISC valve to warm it, then on to another connector on the block under the intake manifold.

If you look closely at the photo of the adaptor, you will see inside it a piece of metal. The opening on the AFM is rectangular, and smaller than 3". That's tha correct adaptor shown.
