So the pump is between the valve cover ports and the intake?
That must be the part I'm missing.
How long can the pump handle drawing oil vapor though it?
So you didn't remove the EGR you just removed some vacuum lines?
As for the idle/die situation - Check your fuel injectors you really don't want an explosion coming from them. Check your fuel pressure. Check your ISC. Check for vacuum leaks.

You can see on the first link where the coolant hose from tb to isc is.
The coolant connection from the bottom of the isc attaches to the intake manifold, at head, on #17111G on this link

the vsv's attach to the under side of the intake manifold with the vacuum hardline assembly rail. There is probably a pic around here somewhere, I'll look later.
edit: I take that back. Originally the fuel vsv mounts way up under on the head or block but if you've...
Mine has not backed out. It is all the way in, stock screw, and I still have the bottom support brace for the afm. No loctite at this time. I don't know what would happen if your setting is somewhere in the middle or all the way out.

It's a California thing.
Code 71 if it's not attached or if something is wrong with your egr system.
You may have a resistor in it's connector or a non cali ecu.
