i took everything(disc, pilot bearing, throwout bearing, and pressure plate) to my clutch builder and he said everything was in great shape..... So the only other thing he felt it could be was the tranny.

well my case is a bit different..... I have been boosting 14psi on the w58 with a 6 puck ceramic disc and a heavy duty pressure plate for a year and a half. I am not 100% sure that its the tranny yet but i will find out shortly.
