99% of the time it's because your wiring job has been done poorly, this includes a new connector they are like $4 or $10-15 pre-made. .999% bad sensor and .0001% a faulty ECU that probably became bad because of a bad wiring job. Do you have pictures of your wiring job?

I got 20 adapters today for denso style injectors. I test fitted them and they fit well but i did discover I have at least one broken injector clip. I might replace the stock clip so i can use my 440s for smog or i could solder on a pig tail..
