If you have the stock fpr in place and your j tube has not been altered in any way the fpr is not likely (albeit not impossible) to be the culprit as the j-tube's restrictive properties act as a failsafe for a faulty fpr.

Try Checking for a small high pressure only boost leak between the intake manifold and the vacuum brake booster on the inner side of the hose. I had a similar situation that caused boost spikes.

Your afm could be on its way out as well. A couple months ago I got a little carb cleaner on mine and couldn't get over 3500 or make boost maybe a pound or so but it didn't start throwing a code on the dash just when in diag mode.
I had a boost leak coming from the hose between the vacuum power brake booster and intake manifold. Check the Interior bends of your hose for cracks that aren't easy to see if you're not looking.
