what kind of turbo is this

It's almost impossible to visually ID a turbo with any level of accuracy from a photo. With that said, we need to see the compressor side as well as the turbine side if we are going to give you any realistic "educated guesses".
From what I can see in your photos, I can tell you only a couple of details:
1. It's from Precision Turbo (http://www.precisionturbo.net/).
2. That turbo has a .68 aspect ratio on the turbine side.
3. It looks to be T4 flange.
You can start here for some general info on turbos so you "speak the language" when you call Precision Turbo:

They will likely ask you to measure the inducer and exducer on the wheels if there are no identifying marks you can read them off the housings:
