Going from D to R and R to D

I was wondering if there is any way an auto can to go into reverse when the selector is in drive and you are driving along (apart from deliberately doing it)

Is there any tie up with O/D and switching into reverse.

Are any of the linkages/position sensors exposed so they can be tripped by debris on the road.

Just curious to see if it could happen in an autobox.

Is this happening to you? Am i to understand its jumping from d to r? I guess it could if your safety lock is broken, but most of the time it pops into neutral because there is a safety mechanism that keeps it from going into reverse at speed.

Cruising along then accidentally bump it into R. Tranny falls out the bottom of your car... Damn im glad i have a 5speed.
