Look at step #2, that's the adjuster. You push on the spiked wheel, to loosen or tighten the pads, through the hole on the face of the brake drum.
If you've already tried that then either you need new pads, new rotors or they need resurfaced.

or you need to tighten the slack in the lever cable. my '84's cable stretched out after 20yrs so i took the console apart and adjusted it. done.
Shoes, actually, and I'd try to adjust the hell out of them before replacing. IIRC they're like $20 per shoe from Toyota!HyRisk said:

Have you actually measured them? They are not thick to begin with. The minimum thickness for the shoe lining is 1mm (.039"). The standard new thickness is only 2.5mm (.098") so double check that unless you already have.
