Before I run off I'll say none of those codes could be related to a vacuum leak so you should fix them first. I'm always surprised by people who try so solve other problems before correcting codes. The CTS (code 22) alone can have an effect on idle.

That should easy to find. Just check the wiring between the sensor and ECU. As for grounds, be sure the ground from the passenger side of the head to the firewall is OK. Same with the main engine ground. Or run a temporary jumper from the battery negative to one of the O2 sensor's mounting...
What cj said. We keep going back to the same issue and that seems to be the engine is remaining in open loop due to an O2 sensor or wiring problem Fix that first.

Stick with it. I know you're trying and that's good. It's a learning experiment. We can't do much from afar and it also helps a great deal to have the right test gear but there are usally ways to compensate for that. You just have to think harder ;)
There seems to be a lot of owners who rebuild engines without any experience. Fwiw this isn't wise. You should have someone experienced helping the first few times you do it.
I dunno...something is goofy there. That aside, you were asking about the 0-5 volt user input, not the sim input. I'd get that WB controller checked out. Again, imho you should install another bung and use the WB only to monitor. Leave AF compensation to the stock O2 sensor.
