R154 Clutch fork hook, broken bolts.

So the other day I was driving my car and when I pressed the clutch pedal to shift, I heard a crack and the pedal went dead to the floor. Upon further inspection I found that the two bolts holding the clutch fork hook had sheared off. I have yet to remove the transmission but, it looks like only the slave cylinder was damaged. The car is basically stock besides an exhaust and a 57 trim and all clutch components were replaced when the transmission was rebuilt about 4000km ago.

So basically my question question is why did this happen or has anyone ever at least heard of this happening?

Agreed, or possibly loose. If they were loose the bracket would wiggle back and forth and slowly wear at the bolts. All things considered, over-torqued is more likely as this would give more of a shearing affect over what loose bolts would give you.
