Yes, If you mean a w58 into a MK4 2JZ supra. Most MK4 2JZGEs come with a w58. If it is a w58 from a MK3 supra, you will have to swap bellhousings to get it to work with a 2JZ.

Yes, NA MKIV's came with a W58, so did 5 speed SC300's...

One from an SC300, MKIV Supra N/A or IS300.

Also, the tail housing/shifter extension is needed from a MKIV Supra N/A.
